Job Description: We are seeking a dedicated and organised Out of Hours (OOH) Care Coordinator to join our team in Spennymoor. The OOH Care Coordinator will be responsible for managing and coordinating care services during weekends, ensuring that our clients receive continuous and reliable support.

Person Specification:

Key Responsibilities:
  • Coordinate and manage care services during weekends.
  • Respond to emergency calls and provide immediate assistance as needed.
  • Schedule and allocate care staff to meet client needs.
  • Maintain accurate records of all care activities and communications.
  • Liaise with clients, families, and healthcare professionals to ensure seamless care delivery.
  • Provide support and guidance to care staff during out-of-hours shifts.

Essential qualifications:

Requirements:
  • Previous experience in a care coordination or similar role.
  • Excellent organisational and communication skills.

  • Proficiency in using care management software and other relevant tools.
  • Availability to work weekends.
